INFINI-T MISSION

The Infini-T program is devoted to providing access to high quality, comprehensive, age- and developmentally-appropriate, culturally competent, and effective HIV and transgender healthcare services for young transgender women, while simultaneously facilitating a strong and clear voice for transgender healthcare equality by providing services that affirm and empower trans and gender variant communities throughout New York City.

WHAT WE DO

In 2012 the HEAT Program received a Health Resources and Services Administration Special Project of National Significance grant to launch the Infini-T Program. Infini-T, a culturally competent, linguistically accessible, population-based, multidisciplinary care model demonstration project, to serve HIV+ and high-risk HIV- transgender (TG) young women of color (TYWOC) in Brooklyn. By expanding HEAT's partnership with the Hetrick Martin Institute (HMI) and utilizing our multidisciplinary transgender focused team including social work, case management, peer advocacy, mental health and medical providers to address the multiple challenges young transgender women face, Infini-T will maximize HEAT's effectiveness in identifying, engaging and retaining young transgender women in care.



INFINI-T PROGRAM GOALS
  1. Ensure early identification of new HIV cases among TYWOC
  2. Ensure timely entry into co-located TG and HIV care
  3. Provide early engagement in mental health services for HIV+ and HIV- TYWOC with unmet mental health needs
  4. Increase the retention rates of TYWOC already receiving HIV medical care
  5. Re-link to care HIV+ TYWOC who were previously in care, but lost to follow-up
  6. RetainHIV- TYWOC with significant risk factors for acquiring HIV in TG care at HEAT
  7. Pilot a group-level psycho-educational intervention, "Just One Of The Girls" (JOG), aimed at improving retention in care for both HIV+ and HIV- TYWOC
